Guv launches MIS portal for vax cards with QR codes

Gov. Eugenio Jose Lacson launched the Negros Occidental Monitoring Information System Portal where fully vaccinated Negrenses can download their COVID-19 Vaccination QR (Quick Response) identification card, Tuesday afternoon, October 19, Provincial Administrator Rayfrando Diaz said.

Those whose names have already been uploaded into the system can download their cards at, he said.

Negros Occidental mayors are urged to ensure that the lists of their fully vaccinated residents are uploaded so they can avail of their cards with QR codes, Diaz said.

He said this will be an easier way to distribute the vaccination cards since more than 300,000 Negrenses have already been vaccinated.

To access the vaccination card at the site one must type in one’s name, date of vaccination and birth date.

“If your name and details do not appear in the system, you have to follow-up with your LGU to upload it to the province,” Diaz said.

The vaccination cards with QR codes will enable a fully vaccinated individual to travel without a negative RT-PCR test result, Diaz said.*
